Jenny from Monash Health Advance Care Planning Service will explain how planning ahead for your future medical treatment can help you.

This session will include information on:

  • Appointing a medical treatment decision maker
  • Completing an advance care directive
  • Making an enduring power of attorney
  • Appointing a support person.

A morning information session on this topic is also being held at Clayton Library on 3 October 2024.
